Patient Registration

Before you come to Foundation West Houston Surgical Center, your surgeon will review with you the details of having your surgery performed at the Center and you will be given detailed instructions. The following forms may be completed prior to your scheduled procedure.

Anesthesia

Anesthesia services for your surgery will be provided by Gulf Anesthesia, a professional corporation consisting of Board-certified Anesthesiologists and Certified Registered Nurse Anesthetists. This service will be billed separately.

Pathology

Pathology services are provided by Lab Corp, a professional organization consisting of board-certified pathologists. Pathologists analyze tissue specimens obtained during some surgeries. This service will be billed separately.

Payment is due on the day of surgery. You will need separate checks for the facility and anesthesia.
